Model withdrawal form

1. When to use this form

This form is for consumers who purchased a paid SmartWerk subscription and wish to withdraw within the 14-day cooling-off period where applicable.

For digital services that start immediately after payment, withdrawal may be limited once you use the service - see our terms (/terms), section 5.

2. Model form (complete and email)

Send the completed form to support@smartwerk.nl with subject: SmartWerk withdrawal.

- I/We (*) hereby give notice that I/We (*) withdraw from my/our (*) contract for the purchase of the following service:

- Order received on (*) / Service started on (*):

- Name of consumer(s):

- Address of consumer(s):

- Email address used for SmartWerk account:

- Date:

- (*) Delete as appropriate.

We will confirm your withdrawal by email within a reasonable time. Refunds are processed via Stripe where applicable by law.
